MTV just did a survey where they asked people what videos broke all of the rules.
Here's the Top 10, with three from Madonna.

how about Smack My Bitch Up by Prodigy, where the whole music video is in first person of a person partaking in various night life activities(including gettin it with a woman) only to turn the camera in the end and realize that it is a girl all along...

actually, the list in question is of the most GROUND BREAKING (aka innovative) music videos. however, MTV has a seperate list call the most controversial, and includes Prodigy's Smack My Bitch Up at number one, and Nas and P.Diddy's video at number five.

this list fucking sucks... none of these are shocking at all... and i agree with Tyler who pointed out why The Prodigy "Smack My Bitch Up" wasnt included here... the freakin thing was only allowed airplay for two days in its uncensored form...

What about music videos from Cradle of Filth?
THere is a music video from Gorgoroth - Carving the Giant, in which they are in concert and there are four blood covered bodies (2 male, 2 female) in crucified fashion, again drenched in blood, fully naked. WIth 100 sheeps heads and carcasses strewn everywhere. This was an actualy concert. THat and its black metal which offends people no matter what.
Or for a better known example, Sweet Dreams - Marylin Manson? Come on people
